Saint Alphonsus Outreach Program

Outpatient Clinics

This service is provided by Saint Alphonsus Health System to help patients better understand and manage heart failure (HF). The program is designed to supplement existing primary care services, utilizes Remote Presence technology, and is flexible depending on partnering facility’s needs. Options include:

  • One-on-one scheduled patient appointments with the clinical nurse specialist, including assessment of vitals
  • Sequence of classroom education sessions for patients
  • Classroom education for nurses

Patient Education Topics

  • Month 1:
    • Taking Control of HF
    • Following a Low Sodium Diet
  • Month 2:
    • HF Medications
    • Self-Care: Following Treatment Plan & Dealing with Symptoms
  • Month 3:
    • Exercise & Activity
    • Managing Feelings about HF
    • Tips for Family & Friends
  • Month 4:
    • Lifestyle Changes
    • Advance Care Planning
    • Heart Rhythm Problems

Classes are approximately an hour each: 45 min. presentation followed by 15 min. questions and answer session.

Outpatient Telepsychiatry Clinic

At the Outpatient Telepsychiatry Clinic psychiatrists act as consultants and work in conjunction with the primary care providers at partnering facilities. The program was designed to improve access to psychiatric services. The Clinic utilizes Remote Presence technology and/or videoconferencing technology.
